The forms listed below will assist you in planning for your study abroad program.

Initial Advising Form

The Initial Advising Form helps our Education Abroad Advisor, Emily Krueger, understand your interests and information before you come in for an appointment. After you set up an appointment, you can fill out Initial Advising Form.


Pre-Departure Checklist

Use this checklist to make sure that you have completed all of the necessary steps before studying abroad.

Health Forms

Please read through the Health Form Requirements form for more information on each form listen below. These forms are a requirement for you to participate in any education abroad program.

Course Approval Form

Use the course approval form to ensure that the classes you take abroad, will transfer back to UNO credits. You will fill out the study abroad course title and number, and then have your academic advisor help you find a UNO course that is equivalent. After this form has been signed by your academic advisor, you can turn it in to the Education Abroad Office.

Budget Form

This form is designed to help you create a realistic budget for your study abroad experience. It is for your personal use and does not need to be turned into the Education Abroad Office or Office of Financial Support and Scholarships.

Financial Aid Form

If you are planning to apply loans, grants, or scholarships to the cost of your study abroad program, you must complete this form and return it to the Office of Financial Support and Scholarships.
