University of Nebraska at Omaha MavCARD Services Cardholder Agreement

Definitions

This agreement governs the use of the University of Nebraska at Omaha’s official identification card, known as the MavCARD. The terms “cardholder” and “cardholders” refer to individuals whose names and images appear on a MavCARD. The terms “University” and “we” refer to the University of Nebraska at Omaha. The term “card” refers to the MavCARD itself.

Eligible cardholders include students, faculty, staff, and affiliates. A student is defined as any individual officially enrolled in courses at the University, whether full-time, part-time, undergraduate, graduate, or non-degree seeking, including those residing in university-operated housing facilities. Faculty and staff include all individuals employed by the University in full-time, part-time, or temporary positions, including academic faculty, administrative personnel, and support staff. Affiliates include contractors, visiting scholars, emeritus faculty, sponsored guests, volunteers, and others with an official relationship with the University who are authorized to receive a MavCARD.

Use and Ownership of the Card

The MavCARD is the property of the University of Nebraska at Omaha and serves as an official university identification card. It is issued to eligible individuals for the duration of their association with the University and must be protected and maintained in good condition. Cards are non-transferable and are only to be used by the individual to whom they are issued. Cardholders are expected to present their MavCARD upon request while on university property.

Cardholders may not alter the card in any way, including attaching stickers, punching holes, or otherwise damaging it. A non-refundable replacement fee will be assessed for cards that are lost, stolen, or damaged. The University reserves the right to cancel or revoke a card at any time and may require the return of the card to MavCARD Services.

Card Expiration and Renewal

Cards issued to students, faculty, and staff remain valid as long as the individual maintains active enrollment or employment with the University. Cards issued to affiliates may include an expiration date based on the duration of their appointment with the University. Renewal of the card is subject to continued eligibility and the approval of university officials.

Lost or Stolen MavCARDs

Cardholders are required to report lost or stolen MavCARDs immediately by visiting the MavCARD Services Office in person, calling 402.554.2220, or using the MavCARD GET Mobile Application. Once reported, the card will be deactivated to prevent unauthorized use. If the original card is found before a replacement is issued, the cardholder must reactivate their card in person at the MavCARD Services Office and present valid photo identification (See Acceptable Forms of Identification) to verify their identity. Once a replacement card is issued, the original card is permanently deactivated and cannot be reactivated, even if found later.

The University is not responsible for unauthorized use or charges on a lost or stolen MavCARD. Prompt reporting minimizes the risk of misuse.

Replacement MavCARDs

Cardholders looking to replace a lost, stolen, or damaged MavCARD must visit the MavCARD Services Office in person and present valid photo identification (See Acceptable Forms of Identification) to verify their identity. All forms of identification must be the original physical document. Photocopies or photos of identification on a mobile device are not accepted. All replacement cards are issued on site and are not mailed to cardholders.

A non-refundable replacement fee of $15.00 will be assessed for cards that are lost, stolen, or damaged. Payment may be made at the MavCARD Services desk, or for students, applied to the student’s account. The replacement fee will not be refunded if the original card is later found after a replacement has been issued.

Damaged MavCARDs

The MavCARD is the property of the University of Nebraska at Omaha. Damaged cards must be surrendered to the university to obtain a replacement. Damaged cards are subject to the replacement fee. Examples include, but are not limited to:

  • Deep scratches or gouges to the magnetic stripe
  • Holes, cracks, bending, or warping
  • Damage caused by chewing or exposure to washing machines or dryers
  • Deactivation due to contact with electronic devices such as phones, computers, or tablets

The University will not disclose cardholder information to third parties without the cardholder’s written permission, except as required by law. For more information, cardholders should refer to the University’s Photo Release Policy.

Facility and Service Access

Possession of a MavCARD does not guarantee access to all University buildings, services, or resources. Access rights are determined separately by the appropriate University departments or authorities and are subject to change without notice. Cardholders are responsible for complying with all access control policies established by the University.

The MavCARD Services Office does not manage or assign building access. Any questions or issues related to building or facility access must be addressed between the cardholder’s department and the University’s Public Safety department who manages access.

Misuse and Non-Transferability

The MavCARD may not be loaned, altered, duplicated, or used by anyone other than the individual to whom it was issued. Misuse of the MavCARD may result in the suspension or revocation of card privileges and may also lead to disciplinary action in accordance with University policy.

MavMONEY

Declining Balance Account

MavMONEY is a prepaid declining balance account linked to the MavCARD and may be used at participating locations on and off campus. The funds deposited are non-transferable and do not accrue interest. Acceptable methods for adding funds include cash, check, or major credit card payments made in person at the MavCARD Services Office, through MavMONEY Cash-to-Card machines, or via the MavCARD GET Mobile Application. Transactions will be denied if there are insufficient funds in the account.

Should a deposited check be returned, a $30.00 returned check fee will be charged. If a credit card transaction is rejected, the amount will be removed from the account. In such cases, the University reserves the right to place a hold on the account until the issue is resolved.

Account Activity, Refunds, and Inactivity

Funds are debited from the MavMONEY account only for authorized transactions. While printed account statements are not issued , transaction history is available upon request. Cardholders must report any suspected errors within thirty (30) days of the transaction date.

Accounts with no activity for a period of two (2) years will be deemed inactive, and the balance forfeited to the University. Upon permanent separation from the University, cardholders may request a refund of their remaining MavMONEY balance if it exceeds $15.00. A $10.00 administrative fee will be applied to the refund. Refunds will be issued by check and mailed to the cardholder’s permanent address within two (2) weeks. Refunds will not be processed for balances under $15.00, unless otherwise required by law.

Dining Dollars

Declining Balance Account

Dining Dollars are a prepaid declining balance account linked to the MavCARD and may be used at participating on-campus dining locations that accept the MavCARD as payment. The funds deposited are non-transferable and do not accrue interest.

Dining Dollars are automatically loaded onto the MavCARD of eligible resident students at the beginning of each semester. Resident students may also purchase additional Dining Dollars during the program period. Non-resident students are not automatically enrolled but may choose to participate by purchasing a Dining Dollars plan through the online portal during the program period. Acceptable methods for purchasing or adding Dining Dollars include payments made through the online portal or by cash, check, or major credit card at the MavCARD Services Office.

Account Activity and Refunds

Funds are debited from the Dining Dollars account only for authorized transactions. While printed account statements are not issued, transaction history is available upon request. Cardholders must report any suspected errors within thirty (30) days of the transaction date.

Dining Dollars are non-refundable and non-transferable, except as required by law. Any unused Dining Dollar balance at the end of the program period will be forfeited.

Program Changes or Discontinuation

The University reserves the right to amend or discontinue the MavCARD program or the associated MavMONEY and Dining Dollar declining balance programs at its discretion. If the programs are terminated or their administration is transferred to another entity, cardholders will be notified and given an opportunity to request a refund or transfer of any remaining balance. Any unclaimed balances will be forfeited to the University after two (2) years for MavMONEY and after the end of the program period for Dining Dollars. Both shall become the property of the University.

Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of Nebraska. By accepting and using the MavCARD, the cardholder agrees to the terms and conditions set forth herein and as amended from time to time.

Amendments to this Agreement

The University reserves the right to update or amend this agreement at any time. Changes will be posted in the MavCARD Services Office, on the official MavCARD website, or communicated through other reasonable means. Amendments become effective upon posting.
