This service is not meant to circumvent or supersede established reporting methods, such as communicating with a supervisor, but rather serves as an additional reporting method for individuals to report their concerns.

How to Report an Incident or Concern

To report a campus-related incident or concern, please fill out the form linked below.

  • When a report is filed, it will be routed to an appropriate university contact
  • Reports will be handled as promptly and discreetly as possible
  • Facts will only be made available to those who need to investigate and resolve the matter
  • The university does not tolerate or condone retaliation toward individuals making a report of concern in good faith
