Required Health Steps After Admission
Health requirements are separate from the admissions process.
Being admitted to UNO is not dependent on submitting immunization records. If you are required to complete health requirements, you will do so after you have been admitted and before enrolling in classes.
Who Needs to Submit Immunization Records?
Required
You must submit documentation of your Measles, Mumps, and Rubella (MMR) immunizations if you are:
- An international student
- A homeschooled student
Not Required
If you are a domestic student who graduated from a U.S. high school, you do not need to submit immunization records to UNO.
International Students: Tuberculosis (TB) test
In addition to the immunization requirements above, international students must provide documentation of a negative tuberculosis (TB) test.
We understand that obtaining a TB test before traveling to the United States may not be possible. If you are unable to complete the test before arriving, you may schedule an appointment with the Nebraska Medicine UNO Health Center during your first term at UNO to have the TB test completed on campus.
Please complete this requirement as soon as possible after arriving at UNO to avoid a hold that could impact future enrollment.
MMR Documentation Requirements
Students who need MMR immunizations or a rubeola blood titer because they cannot find their records can call 402.554.2374 (please dial the full 10-digit number whether on or off campus) to schedule an appointment at Nebraska Medicine - UNO Health Center.
- Please note immunizations or laboratory services will incur a charge that can be billed to insurance. Students can also contact their preferred health care provider to receive this service.
Students required to submit records must provide:
- Proof of two MMR vaccinations
or - A positive rubeola (measles) blood titer
Include the existing bullet points regarding dates, name, birthdate, English translation, etc.
Immunization records must:
- Show proof of two separate MMR shots showing the month, day, and year it was given OR students can submit a copy of a positive rubeola blood titer lab result showing immunity. The first shot must have been received no earlier than a student’s first birthday, and the second shot must have been received at least 28 days later.
- Include the full legal name of the student and the student’s birthdate.
- Be in English
Immunization records can often be found at:
- Previous high school
- Previous university/college
- Health care provider’s office
- Public health department
- Military record
- Nebraska residents and individuals who were immunized in the state of Nebraska may be able to access their records online through the Nebraska State Immunization Information System (NESIIS).

Immunization Waiver
Please be advised that UNO only considers waivers for individuals with a sincerely held religious belief, practice, or observance that conflicts with the MMR Immunization Requirement. You may request a religious accommodation by visiting the immunizations page.

Optional: Power of Attorney (POA) Form (for 18-year-old students)
The age of majority in Nebraska is 19, meaning parental permission is required before a student 18 or younger can receive health care at Nebraska Medicine UNO Health Center. It is strongly recommended that parents or guardians of 18-year-olds fill out and have notarized a Power of Attorney form. POA forms can be dropped off, mailed, or faxed to the UNO Health Center front desk, located at 102 H&K. For more information, call the UNO Health Center at 402.554.2374. Please note POA forms are not valid for 17-year-olds.
