Health Requirements

Students born on or after January 1, 1957, must show official proof of receiving two (2) MMR shots (measles, mumps, and rubella). Future registration may be withheld if this requirement is not met.

If you plan to transfer to another NU institution, it is important to reach out to the admissions department of the respective institution for guidance on this requirement.

Immunization Religious Waiver

Please be advised that UNO only considers waivers for individuals with a sincerely held religious belief, practice, or observance that conflicts with the MMR Immunization Requirement. You may request a religious accommodation by completing and submitting the Religious Accommodation Request Form to the Division of Student Life and Wellbeing or by emailing the form to unostudentlife@unomaha.edu.

Optional: Power of Attorney (POA) Form (for 18-year-old students)

The age of majority in Nebraska is 19, meaning parental permission is required before a student 18 or younger can receive health care at Nebraska Medicine UNO Health Center. It is strongly recommended that parents or guardians of 18-year-olds fill out and have notarized this Power of Attorney form. POA forms can be dropped off, mailed, or faxed to the UNO Health Center front desk, located at 102 H&K. For more information, call the UNO Health Center at 402.554.2374. Please dial the full 10-digit number whether on or off campus.

Please note POA forms are not valid for 17-year-olds.

1 dose every 5 to 10 years Every student should get one booster dose of Tdap.
Meningococcal 1 dose after 16 years of age College students in close contact are more susceptible.
Hepatitis B 3-dose vaccine series Serious viral disease affecting the liver.
Hepatitis A 2-dose vaccine series Viral disease affects the liver. College students traveling outside the U.S. needed.
Human
Papillomavirus
(HPV)
3-dose vaccine series Can prevent most cases of cervical cancer and genital warts. Recommended for both females and males 18-26 years of age.
