This form is for anyone to submit information regarding stormwater quality issues on any UNO campus.

This information will be used by EHS to follow up on stormwater related illicit discharge and/or spills, construction site complaints, or illegal dumping.

Please immediately report anything that may cause direct or substantial harm/distress to the environment (e.g., petroleum spill, fish kill).
