All reports of injury must be made using the Employee Incident Report Form.
This form must be completed, reviewed with a supervisor, and submitted to Environmental Health and Safety (EHS) within 24 hours of the incident.
All reports of employee injuries (faculty, staff, student workers) must be made using the UNO Employee Incident Report Form [unomaha.edu].
