Medicaid Application and Assistance

Nebraska Department of Health and Human Services (DHHS)

About: Medicaid is both a federal and state service that helps with medical costs for low income and disabled individuals and families.

Nebraska Medicaid Health Plan Manager

Heritage Health

Eligibility/Requirements: Must be a current Nebraska Medicaid and/or CHIP recipient.

Overview: Heritage Health is a health care delivery system that combines Nebraska's current physical health physical health behavioral health, and pharmacy programs, into a single comprehensive, and coordinated system for Nebraska's Medicaid and CHIP clients.

About: As of January 1, 2017 Heritage Health manages the Nebraska Medicaid Program. Medicaid provides health care services and insurance to eligible elderly and disabled individuals and eligible low-income pregnant women, children, and parents. CHIP is part of Nebraska Medicaid and provides medical assistance to children age 18 and younger whose households meet the income guidelines. Heritage Health offers three health plans for Medicaid recipients. Upon approval for Medicaid, a plan is chosen for the Medicaid recipient by Heritage Health. The Medicaid recipient may then change to one of the other two plans within the first 90 days. To apply, an application must be completed and given to the Nebraska Department of Health & Human Services. You may download an application form from the website and mail it to the nearest Department of Health and Human Services Office. Approval can take up to 3 months

United Healthcare

Eligibility/Requirements: United Healthcare offers two health plans in Nebraska.

Dual Eligible Plans are designed for those who meet the requirements for both Medicaid and Medicare Parts A and B; and Medicaid only plans.

Cost/ Payment: Costs vary but can be as low as $0 per month.

About: Provides an array of plans for:

  • Pregnant mothers and their babies.
  • Children up to age 19.
  • Adults and families in times of financial hardship.
  • People living with disabilities or other serious health conditions.
  • People who need help to live independently.
  • Seniors who qualify for both Medicare and Medicaid.

WellCare of Nebraska

Eligibility/Requirements: Must be a Medicaid member.

About: WellCare of Nebraska is a managed care plan that has a contract with the Department of Health and Human Services to give Medicaid members access to quality, cost-effective health care.
