Eligibility: Persons suffering from a serious medical condition or injury resulting in long-term disability.

Cost: Free case evaluations and Contingent-fee services. There is no cost to you for representation unless or until Disability Claims Consultants successfully helps you obtain benefits.

About: Provide personalized assistance for people with disabilities seeking Social Security Disability (SD) benefits or Supplemental Security Income (SSI). Even if you have tried and failed to receive benefits through the government, you can and are encouraged to seek assistance through Disability Claims Consultants. Respite and Child Care Services Through CRCC Include:

Weekend Program

Eligibility: Children with medically fragile conditions, developmental delays and other physical challenges and their siblings aged 6 weeks to 21 years. Guardians must schedule in advance.

About: CRCC’s Weekend Respite program was designed to give families a break from the intense structures of the weekly routine and provide a unique and relaxing experience. The weekend program is a licensed Respite Care Service by the Nebraska Department of Health and Human Services.

Hours: Saturdays 7:00 a.m. to 9:00 p.m. Children can attend all day or your preferred hours.

Birth to Three | Early Childhood Education Program

Eligibility: Children birth to three years of all abilities with medically fragile conditions, developmental delays and other physical challenges

About: Bringing children of all abilities together to learn in an early childhood environment that is centered on excellence, joy and inclusion. Children at CRCC are presented with rich experiences that teach them empathy, compassion, understanding, patience, diversity and more.

Includes quality services that prepare children for long-term success such as integrated therapy services, individualized education programs, early intervention services and more.

Hours: Monday-Friday 7am-6pm

Where? Both CRCC locations

Learning Together Preschool

Eligibility: Children at least 3 years old and no older than 5 years old by August of the enrollment year with medically fragile conditions, developmental delays and other physical challenges

Cost: $185 enrollment fee + $185/week (for typical developing children): includes breakfast, lunch and two snacks each day.

About: Promotes learning through purposeful play, and choice based activities that are relevant to the child and their development.

Hours: Monday-Friday 7am-6pm

The Core of Hope Before and After School Program

Eligibility: School aged children with medically fragile conditions, developmental delays and other physical challenges

About: Full or part time care for before and after school and during school breaks.

Provides for medical and educational needs of clients, in an inclusive and loving environment focused on building relationships, gaining independence and instilling in children a sense of responsibility for themselves and their surroundings. Daily activities encourage positive peer interactions, communication skills, daily living skills, responsibility and what it means to be part of a community.

Summer Camp

Eligibility: School aged children up to 21 years old with medically fragile conditions, developmental delays and other physical challenges and their siblings.

Cost: $50 one-time non-refundable per child. Accepts Title XX payment

About: 11 week camp with weekly in-house activities, special guests and fieldtrips.

Madonna Rehabilitation Hospitals (MRH)

Eligibility: Persons of all ages in need of intensive rehabilitative therapy for traumatic brain injury, spinal cord injury, complex medical and or other serious injuries.

Cost: MRH accepts most insurances and private pay. Their Financial Assistance Program is available for low income residents who are uninsured or underinsured and do not have adequate financial resources to pay for necessary healthcare services provided by MRH.

Shared Living Provider (SLP)

Through The Community Supports Network (CSN)

Cost: Varies. Resident expected to contribute payment for room and board.

About: A unique community-based living option. A contractor provides supports to a person who comes to live in their home, and the residence effectively becomes the person's home. Residents contribute to costs via a room and board payment. Additionally, the subcontractor is paid to support the person to live and become fully integrated into community life. Supports include formal training, based on the person’s identified goals, as well as development of interests and connections that enable the person to be a fully participating and contributing member of the community.

CSN Overview: This non-profit organization supports people with mental health needs or developmental disabilities to gain skills, knowledge and experience to increasingly use and benefit from resources and settings available to all citizens in our community. They believe every person has value and deserves to be treated with respect and dignity.

About: Provides funds for non-medical services such as: respite services, mileage, meals and lodging for long-distance medical trips, special equipment, home modifications, sibling care during medical appointments and training for children with disabilities.

The Hearing Improvement Kids Endowment (HIKE) Fund

Eligibility: Children with hearing losses between the ages of newborn and 20 years with a financial need.

About: Provides grants for children who have hearing aids and/or an assistive learning device. Grants are used to cover or lessen the costs of hearing aids, enclosed caption converters for television, tactile units, FM units (into which a hearing aid is plugged for amplification), computers to assist deaf children in communicating, as well as other technical devices as needed.

Medically Handicapped Children’s Program (MHCP)

Eligibility: Children aged birth through 20 years of age who are Nebraska residents.

Must be diagnosed with an eligible chronic medical condition and an active individual medical treatment plan (MHCP- Medical Conditions)

Must meet financial eligibility based on current regulations

About: This program provides specialized medical services for children and youth with special health care needs. Services may include service coordination/case management, specialty medical team evaluations, access to specialty physicians, and payment of authorized medical services.

Client Assistance Grants

Through the United Cerebral Palsy of Nebraska (UCPN)

Eligibility: Individuals and families of individual with cerebral palsy or conditions with similar effects that have limited financial resources or do not qualify for other financial support. Applicants may be asked to proide proof of income.

About: The Client Assistance Grant program helps with things such as purchasing adaptive equipment, removing barriers in the home and providing scholarships for special programs like summer camp, sports programs and training. UCP of Nebraska will also assist in identifying and acquiring other sources of funding.

Eligibility: There are no qualifications or income restrictions to use HEKP's service

Assistive Technology Partnership (ATP)

Eligibility: All resources have their own eligibility requirements which may include age, residence, disability, income, request, and more.

Cost/Funding: Many vary based on service and available resources. After completing a Service and Device Application, an ATP Resource Coordinator will assist you in finding resources available to you.

About: Free and low cost services that offer equipment or devices helping individuals safely complete a task and as independently as possible, e.g. communication board, ramps, lifts, bathroom modifications, etc.

ReUse Network Through ATP

About: Provides recycled, reused, or repaired durable medical equipment (DME), adapted devices, or assistive technology.

Equipment

Demonstrations allows individuals and groups to compare the features and benefits of various types of assistive technology.

Short-term Equipment Loans allows you to try equipment out before you buy it to make sure it is the right fit for you.

Borrowing equipment Can fill gaps while you are waiting for a repair or shipment of your own equipment and on a temporary basis while you recuperate from an injury or illness.

Home Modifications Through ATP

About: Technology Specialists work with individuals who have a disability to determine what assistive technology/home modifications are appropriate for their primary living environment. Modifications may include grab bars in the bathroom, entrance modifications, and assistive technology such as automatic door openers, stair lifts, and vehicle modifications.

Work and School Modifications Through ATP

About: Helping employees, employers and students find resources and determine appropriate technology to help you be successful.

Services at School

Eligibility: ATP offers separte programs for students and children aged Birth- 3 years, and 3-21 years.

About: ATP Partnership Education program provides services to IFSP and IEP team members working with children and students with disabilities who are receiving services within school systems.

Services include: technical assistance for phone, email and online video (e.g. Zoom) technologies; Training for IFSP/IEP teams and school personnel; Access to a Statewide Assistive Technology Equipment Loan (AT4All.com) for up to 60 days at no cost.

The National Deaf Blind Equipment Distribution Program:

Eligibility: Persons with significant combined vision and hearing loss who meed federal income and income guidelines.

Cost: Free or low cost for eligibility participants

About: Also known as iCanConnect, is a federally funded program coordinated by ATP in Nebraska that can provide telecommunication equipment and training to stay connected with family and friends.

Shaken Baby Syndrome

About: Shaking a baby can result in lifelong disabilities, such as brain damage, seizures, paralysis, and even death.The number one reason a baby is shaken is due to frustration from non-stop crying.

If you find yourself being overwhelmed: Place the baby in a safe place, like a crib, and leave the room; slowly count to ten while taking deep breaths; call someone to take over for awhile; and you can contact the 24/7 crying baby helpline at (886) 243-2229.

Secondhand Smoke

About: smoking around infants and children can cause serious health problems and risks to their developing bodies.

Thirdhand smoke: refers to the toxins from cigarette smoke that stick to soft surfaces and can harm babies.

If someone is smoking near your baby or child don't be afraid to speak up. Your voice can make a difference in your child's life.

Sudden Infant Death Syndrome (SIDS)

About: SIDS is the sudden and unexplained death of an infant who is younger than one year old. You can help prevent SIDS by taking the right precautions

Tips for reducing the risk of SIDS: Always place your baby on their back to sleep during naps and bedtime on a firm mattress with a fitted bed sheet. If your child falls asleep in a swing, car seat, etc. safely remove them and place them on their back in their crib.

Do not use bumper pads, blankets, pillows, or stuffed animals in the baby's bed.

Do not smoke, drink alcohol or use drugs while pregnant or around the baby.

Schedule and attend regular checkups with your baby's doctor.

Co-Sleeping

About: Sometimes called "bed-sharing" means babies and their caregivers sleep together in the same bed.

Why it is Dangerous: Co-sleeping can cause physical injury and suffocation to a baby or young child and may prevent caregiver from getting a good night's sleep.

Eligibility: Individuals of all ages who have a disability. Adult supervision required. Must register for events.

About: Life can be filled with endless appointments and chores – life should be fun! Family Focus is designed to bring families facing similar challenges together to draw strength and knowledge, as well as relax and socialize.

UCPN Overview: Strives to advance the independence, productivity and full citizenship of people with cerebral palsy and similar disorders.

United Cerebral Palsy of Nebraska serves the condition, as well as serves people and adults with cerebral palsy. They also work with individuals who have other disabilities as well as family members. They offer financial programs that anyone is able to attend, but also some that are inclusive to people with cerebral palsy.

Adaptive Sports Program Through the University of Nebraska Omaha (UNO)

Eligibility: Students at the University of Nebraska Omaha

About: The Adaptive Sports Program introduces student athletes to the game of wheelchair basketball through a competitive environment that stresses goal setting, teamwork, sportsmanship and socialization in a team basketball atmosphere.

Sports and Leisure Program Through CHI Health

Eligibility: Youths and adults with physical disabilities and their families and friends. All skill levels are welcome!

Overview: The CHI Health Rehabilitation Institute's Sports and Leisure Program gives people with physical disabilities the chance re-learn a recreational skill or try something completely new! Programs are offered two to four times a month at various locations. Examples of programs include: fishing, zoo trips, playhouse, baseball games, movies, martial arts, archery, and more.

Grant: The Hearing Improvement Kids Endowment (HIKE) Fund

Eligibility: Children with hearing losses between the ages of newborn and 20 years with a financial need.

About: Provides grants for children who have hearing aids and/or an assistive learning device. Grants are used to cover or lessen the costs of hearing aids, enclosed caption converters for television, tactile units, FM units (into which a hearing aid is plugged for amplification), computers to assist deaf children in communicating, as well as other technical devices as needed.
