State Tournament Forms

Coaches must complete and submit the following forms before teams can compete in the state tournament. (Invitational Directors may use different forms. Please check with them or check their websites for more information.)

Please submit your forms by one of the following methods:

• Bring them with you to the tournament
• Submit them via email to deannajohansen@unomaha.edu
