PROPOSAL SUBMISSION FORMS

Proposal Request Form
(PRF Page 1) (PRF Page 2)- The PRF form is required for every proposal submitted to an external sponsor or agency.

Disclosure of Interest Form
Required for proposals with budgets of more than $5,000.

F&A Form - Required for every proposal.

Type of Request Definitions (PRF page 1):

Instruction includes all funds expended for activities that are part of an institution's instructional program.  

Basic research is undertaken primarily to acquire new knowledge without any particular application or use in mind.

Applied research is conducted to gain the knowledge or understanding to meet a specific, recognized need.

Development is the systematic use of the knowledge or understanding gained from research directed toward the production of useful materials, devices, systems, or methods, including the design and development of prototypes and processes.

Public Service includes all funds expended for activities established primarily to provide noninstructional services beneficial to individuals and groups external to the institution.

Student aid/Traineeship applies only to funds given in the form of outright grants and trainee stipends to individuals enrolled in formal coursework, either for credit or not.

Detailed PCS Code Definitions

These forms require the signatures of the project director(s) and appropriate chair(s) and dean(s). Bring your proposal and signed forms to SPR at least 3 working days prior to submission deadline.

INVENTION DISCLOSURE FORM

Invention Disclosure Form
A UNO Invention Disclosure Form must be completed when an individual has developed a new technology using University facilities or resources. Contact the Director of SPR for assistance in completing this form.
