LICENSED CLINICAL SOCIAL WORKER (LCSW) [CMSW + LMHP]
Qualifying and accredited MSW
+ 3000 hours of post MSW experience within 5 years of date of application under supervision* of an LCSW**** of which at least 1500 hours must be direct (therapeutic or clinical) face-to-face client contact
+ Pass the ASWB Clinical examination

PROVISIONAL LICENSE FOR MENTAL HEALTH PRACTITIONER (PLMHP)
LB622 passed June 17, 1997, and became effective September 13, 1997. It requires a practitioner to have their PLMHP to practice in the Mental Health field and to accumulate post MSW experience. You can hold a PLMHP for a maximum of 5 years for a fee of $25.00.

Even numbered years: September thru December = $27.00
Odd numbered years: January thru August = $27.00
Odd numbered years: September-December = $26.00
CMSW
If the date of issuance falls on:
Even numbered years: September to August of Odd numbered year = $27.00
Odd numbered years: September to August of Even numbered year = $26.00
LMHP
If the date of issuance falls on:
Even numbered years: January or February = $51.00
Even numbered years: March-August = $26.00
Even numbered years: September-December = $52.00
Odd numbered years: January-August = $52.00
Odd numbered years: September-December = $51.00
**A minimum of 1 hour face-to-face contact per week with the appropriate supervisor.
***Hours prior to September 1, 1994 for LMHP can be from Licensed Qualified M.D., Licensed Psychologist, or a CMSW
****Hours prior to September 1, 1994 for LCSW can be from a CMSW

COURSE REQUIREMENTS
The following UNO courses meet the 5 specific curriculum requirements (of 1 credit hour of graduate course work) specified in the Rules and Regulations of the Mental Health Practice Licensure law. In addition to these you must have 300 hours of direct client contact in graduate practica under a qualified supervisor for those who complete the practicum courses after September 1, 1995.
