All forms and guidelines mentioned on the Academic and Student Affairs (OASA) website are cataloged here for quick reference. Even if you are very familiar with a process, please review the policies and procedures for a process before completing any forms. Although redesigned and reformatted, all forms and documents provided on this OASA website are current and historically accurate.

Before completing any forms, please review the appropriate guidelines.

Some forms are provided in .rtf formats to ease online completion with multiple word processing programs. Although unlikely, some formatting features may be lost if used with programs other than Microsoft Word. Once the form appears in the browser window, from the File menu, select Save As and save the document on your hard drive or floppy disk.

Most of the forms are protected, which means that users may enter information only in the spaces provided. Before completing the forms, you will need to unprotect them unprotect the document. To unprotect a document in Microsoft Word, from the Review tab, choose the appropriate seeting under Protect Document.

Some forms are provided in .pdf format. To view these documents, you will need Adobe Acrobat Reader.
